3. Objectives (terminal and enabling)

5. Procedures (introduction/warm up, whole class work, pair work, teacher talk, student talk, closure) 
6. Evaluation
7. Extra-class work

Does this seem like it has all necessary parts? Do you use a different template?  Please let me know if you have something better or any ideas in the comments.
